Home- and Community-Based Services are at Risk of Devastating Cuts!
Dear Representative Dan Newhouse and Senators Patty Murray and Maria Cantwell,
We call on Congress to protect Medicaid Home- and Community-based services (HCBS) and reverse the deep cuts to Medicaid. Without immediate action, millions of seniors, people with disabilities, and families will lose essential care that enables them to live and age with dignity.
These cuts are hitting families hard—especially the “Sandwich Generation” balancing care for aging parents and children. HCBS provides vital support that keeps family caregivers employed and loved ones safe, while protecting essential home care workers—most of whom are women of color and immigrants.
As moms, parents, and caregivers, we urge you to invest in health care and our care economy. We ask that you:
- Reverse the cuts to Medicaid and oppose legislation or funding freezes that add burdensome reporting, restrict program eligibility, or places limits on Home- and Community-Based Services.
- Invest in the home care workforce, ensuring fair pay for caregivers, protections for immigrant care workers, and stability for families.

For families like Janet’s, Medicaid is a literal lifeline. But right now that lifeline is under direct threat. Last year, Republicans in Congress and President Trump passed the “Big Beautiful Bill,” which cut over $1 trillion (yes, trillion with a ‘T’!) from Medicaid, and these cuts are already triggering states to cut back on Home- and Community-Based Services (HCBS)—the in-home care aides, personal assistants, and respite care that allow seniors and people with disabilities to live with dignity at home rather than in nursing facilities.
These cuts are coming at a time when so many of us are struggling to make ends meet. With 95% of Americans agreeing we are in an affordability crisis, families are struggling just to keep up with rising gas and grocery prices¹. Caught in the middle is the “Sandwich Generation,” parents doing double duty caring for an aging family member and their children. Without HCBS to provide everyday needs—like help bathing, dressing, and caregiver respite—family caregiving quickly becomes an unsustainable, more-than-full-time job that forces parents out of the workforce.

Even the bipartisan Congressional Caregiving Advisory Council acknowledges that family caregivers are the backbone of our long-term care system. Yet, as federal Medicaid dollars disappear, states are redirecting funds away from HCBS. In fact, at least 9 states have already proposed or enacted direct cuts, caps, or rate reductions targeting HCBS, with dozens more forecasted to do so².

This is an emergency. To make matters worse, families are facing a severe home care worker shortage. The dedicated care workers who show up for our loved ones—the vast majority of whom are women of color and immigrants—are consistently underpaid, undervalued, and overworked. Cutting Medicaid only deepens this labor shortage, driving care workers away and forcing the people they care for into more costly and unwanted institutional settings. We cannot allow this essential care to be cut—Congress must roll back these cuts to Medicaid immediately!
